Pain may occur only with use or also at rest. Polyarticular joint disorders may affect different joints at different times. Articular sources of pain originate within the joint. The synovium and joint capsule are major sources of pain within a joint.

Pain affecting multiple joints in the absence of inflammation may be due to increased joint laxity and excessive trauma, as in benign hypermobility syndrome. Chronic polyarthralgia in adults is caused most often by RA and osteoarthritis. X-rays are often not helpful early in the disease course. These disorders can manifest with axial involvement. Crystal-induced arthritis is most often monoarticular but sometimes oligoarticular.
